Vendor Documents and Forms
If you are interested in becoming and independent
contractor for Professional Field Services, LLC please
complete the following forms and documents.
The Personal Data and Coverage Area form can be
completed online, the Independent Contractor Application,
Independent Contractor Agreement,
and W9 Tax Form must be printed out and signed and
then scanned and emailed back to PFS to the email address
your recruiter gave you. If you have no recruiter then
submit the Personal Data and Coverage Area form and
one will contact you.
